Privacy Statement  Protected Health Information (PHI)  



Entities that involve the use and disclosure of protected health information under services calibration and maintenance of x-ray imaging equipment. Parker X-Ray may make any and all uses of protected health information necessary to perform its obligation set forth in Attachment A and under the Service Agreement between the parties. Additionally Parker X-Ray may disclose protected health information and only to authorized employees, information for the purposes authorized by this Agreement subcontractors and agents of Parker X-Ray

Parker X-Ray, is potentially exposed to PHI in two distinct ways:
A.)As an installer and service/maintenance provider, service can be peripherally, not intentionally, exposed in the course of performing their responsibilities maintaining x-ray imaging equipment.
B.)Parker X-Ray is potentially exposed when transporting dated x-ray films (purged) to be chanelled to the reclaiming company (recycled).
Under this scenario “B”, Parker X-Ray agrees to channel these records to only HIPAA compliant reclaiming businesses.

Parker X-Ray will require all of its employees, has a policy not to use and/or disclose the protected health information only as permitted or required by this agreement or as other wise required by law.

Parker X-Ray will require all of its employees, has a policy to request the minimal amount of protected health information, which will adequately meet its intended needs.

Parker X-Ray will require all of its employees, has a policy to notify the privacy offer of any disclosure or unauthorized use of the protected health information that is not required.

Parker X-Ray has a policy to use reasonable efforts to maintain the security of the protected health information.

Parker X-Ray will require all of its employees, representative, subcontractors or agents that receive or use or have access to protected health information under this Agreement to agree and adhere to the same restrictions and conditions on use and/or disclosure that apply herein.

Parker X-Ray has a policy to provide technical support information as requested by which may allow Healthcare Provider/CE to respond to a request of the disclosures of the individual’s protected health information.

The Healthcare Provider/CE agrees to notify Parker X-Ray of any changes in the form of notice of privacy practices that it provides to individuals.

This agreement shall become effective on the Effective date and shall continue in effect until all the parties have been met, unless terminated by mutual agreement of both parties.

This agreement may not be modified or amended, except in writing as agreed to by each party.
